To visualize a rotator cuff tear, try the following:

Find an elastic rubber band and a medium-sized rock. In this example, the rock represents the bones in your shoulder and the rubber band represents the muscles that support your arm. Stretch the rubber band out flat on the rock and rub it lightly until it starts to tear in the middle. When you look at the rubber band, you'll notice a frayed area.

If you continue rubbing, the rubber band will eventually snap. This snapping of the rubber band can be compared to a rotator cuff tear.

It takes a fairly significant force to cause a tear with the same dramatic effect that you find with the rubber band and the rock. Luckily, the muscles in our bodies are a lot tougher than rubber bands. Also, our body's muscles know how to heal themselves over time if the injuries are not too severe.

The primary indication of a torn rotator cuff is pain in the muscles surrounding the shoulder and the upper tip of the humerus, or arm bone. There are three types of tears: tendonitis, chronic tears, and acute tears. The type of pain experienced indicates the severity of the tear.

Tendonitis is a condition that can lead to chronic tear. The muscles aren't torn yet, but they have become fragile and tender. The pain associated with tendonitis is deep and dull and is most commonly experienced by middle-aged women.

A chronic rotator cuff tear is characterized by a dull pain that typically gets worse over time. Sharp pain may be felt when the arm is moved at certain angles, generally ranging between 80 and 120 degrees. This type of tear is more common in people who repetitively use their arm for activities that are stressful to the muscles and tendons surrounding the shoulder joint.

An acute tear is characterized by a sharp pain that is felt after a sudden movement of the arm or after being hit in the arm or shoulder. This type of injury is most often experienced by athletes or by people who suddenly exert too much pressure on their arm and shoulder muscles without properly stretching and flexing first.

If you experience any of these types of pain in your shoulder area, you should consider visiting a doctor as you could possibly have a torn one of the rotator muscles. For more severe tears, your doctor may suggest surgery; however, the pain can often be eased with non-surgical methods of treatment.

Rest is the primary method for treating the shoulder pain.

If you overexert your muscles, you may end up enlarging the tear. Anti-inflammatory medications are another option for easing the pain. Wearing a sling can help prevent further damage by restricting the movements of your arm.

If you do find yourself with a torn rotator cuff, do not despair. It does not have to be the life-altering injury that ends your sports career or keeps you from normal functioning as a human. When Bursitis Pain Attacks the Shoulder

Shoulder bursitis is one of the most common causes of upper arm pain. The shoulder bursa can be described as a bag filled with fluid that lubricates and surrounds the muscles and tendons of the shoulder. When inflammation occurs, this is called bursitis. When bursitis pain attacks the shoulder, diagnosis and treatment is required to restore the shoulder back to it's normal state.

The shoulder actually has more than one bursa. The subdeltoid bursa is the largest one, and the one that is most likely to cause a problem. The subdeltoid bursa is also referred to the subacromial bursa. When the subdeltoid bursa becomes inflamed or irritated, it can be difficult to move the shoulder or even perform daily activities that involve use of the arm.

What Causes Bursitis in the Shoulder

Injury is the most common cause of shoulder bursitis. This can be a result of a fall, a blow to the shoulder, or even overuse, such as exercising or repetitive movements that a person is unaccustomed to. Overuse of the shoulder joint and muscles can lead to pain that can be quite debilitating. Sometimes the pain seems to start out of the blue with no recollection of injury or any event to trigger the pain. Inflammation associated with the shoulder tendon is called "tendonitis."

Symptoms Associated with Shoulder Bursitis

With shoulder bursitis, chronic joint pain is the main problem. The pain typically starts out gradual and increases over time. The pain normally is located on the outer part of the shoulder and can sometimes be sudden and severe.

This pain can be felt over the deltoid muscle area around the shoulder. It can also spread down the arm to the elbow or wrist. This bursitis pain can be affected by lying on the affected side or trying to reach above your head.

Some people have what's called a "painful arc of movement." This simply means that there is no pain when the arm is not moving, but they develop pain when attempting to lift the arm. This usually occurs when raising the arm to about 60 degrees of elevation. Once the arm extends beyond the 90 degree point, the pain eases or subsides completely.

Diagnosis and Treatment

Your doctor will perform an exam and a series of manual tests, one of which is called an impingement test. This is to determine whether there is inflammation of the bursa or the tendons.
It may be necessary to perform blood tests and/or x-rays. An ultrasound is most commonly used to diagnosis true bursitis.

There are several treatments options used for bursitis. Your physician may recommend one or all of them.

* Pain medication or anti-inflammatory medications
* Rest the affected area
* Physical therapy performed by a physical therapist, chiropractor, or osteopath
* Securing the shoulder by utilizing a sling or brace
* Acupuncture

Erb's Palsy is a brachial plexus disorder. What does that mean? The brachial plexus is a cluster of nerves where the neck meets the shoulder, on each side. They travel from the spinal cord, between the vertebrae, through each armpit and down into each arm and provide almost the entire arm and hand with movement ability and skin sensation.

When a baby has Erb's Palsy, the most frequent indication is lack of movement in one arm. The arm will be straight down at the baby's side and sometimes it will be a little turned, with the wrist bent and the fingers straight. There may also be a droopy eyelid on the same side if the injury is more severe.

Birth injury

The brachial plexus is sometimes injured during a difficult birth, such as:

· When the baby is in a breech presentation

· When the baby is particularly large

· When labor goes on a long time

· When forceps must be used to help the baby emerge from the birth canal

The injuries are a stretching or tearing of the brachial plexus nerves. The resulting arm disability is usually on one side only, as the baby's neck is stretched while one shoulder is caught against the mother's pelvic bone, so only one brachial plexus is damaged rather than both.

Some birth injuries are avoidable with attentive and responsible medical care. If your baby has Erb's Palsy, you might benefit from some legal assistance in discovering whether or not that injury could have been avoided. Were the forceps used too vigorously? Should a caesarian section have been performed? Could the labor have been shortened? Were attending medical personnel negligent in any way? Two groups of nerves

Types of injury

There are four types of nerve injury, with different resulting disabilities:

1. Stretching - The nerve is damaged but not torn, and this is the mildest scenario, usually healing up by itself within about three months

2. Avulsion - The nerve is torn from the spinal cord, and this is the most severe scenario, requiring treatment

3. Rupture - The nerve is torn, but not from the spinal cord

4. Neuroma - The nerve is injured by pressure from scar tissue which forms after the original injury

When a nerve is injured, regardless of how that happened, the result is numbness and loss of movement.

Treatment

Since stretching is the most common type of brachial nerve injury, and this heals by itself within a few months, no treatment is needed in most cases of Erb's Palsy.

But if no improvement is evident after three months, surgery may be performed, and a splint worn afterwards for about three weeks. This will not immediately solve the problem, because growing at the approximate rate of an inch per month, it will be a while before nerves that are repaired at neck level reach the hand and lower arm muscles.

If the child is older and can follow instructions, a tendon transfer may be done, which moves the tendon attachment. Tendons are the connective tissue connecting muscles to bones. This can help the child with shoulder and wrist movements and hand grip. A cast will have to be worn for about six weeks, and at night, a splint will be worn for about six months. Physical therapy will also be part of this follow-up.

When your baby has been injured, it's painful and baffling for the parents, and it can be hard to know where to turn for help. The thing you need for practical help and support is legal advice, legal representation, and all the paperwork done for you.

Something for you to Ponder!

Before Christmas, I was driving home at dusk on a busy highway with lots of oncoming traffic when a deer ran straight in to our truck. I had both children with me, sleeping in their car seats, only our 21 month old even woke up with the ruckus. It scared the bejesus out of me and all I can remember is legs and eyes rolling over the window and then I knew what happened. I stopped and couldn't control myself from shaking; a nice man who witnessed the accident stopped and checked to see if we were okay.

We walked back to see if, by some miracle, the deer would need help but the only ones in need of help was us. No one was hurt. With this much damage and the fact that the truck was not drivable I called 911 and they informed me that the kids must remain strapped in car seats until the police arrived to take the report, which was over 90 minutes later. Only a mom knows how to keep a toddler entertained while strapped in a car seat for that long. We needed a tow truck as I pretty much tore the front end of the truck off, but to the amazement of the police, the tow truck driver and myself, the air bag didn't even go off. No damage to any other part of the vehicle other than the front end. The insurance company advised us that, because the kids were in the car seats when the accident happened, we would have to forfeit both car seats and would have to replace them. Of course, insurance would cover them up to a specific dollar value. (Don't even get me started about how some people rape the insurance industry with fraudulent insurance claims that cost us all in the end). If the children had not been in them we would not have to replace them. Even though there was no physical damage to the seats, or even close to them, they had been in an accident. This is the law, just black and white. I asked what they would do with the car seats and was told that they would cut the straps and they would be tossed into the garbage. Perfectly good car seats pitched in the landfill.

So recently while in New Zealand, we rented car seats instead of lugging them all across the country and the seats we have now would probably never pass Canadian safety code. My daughter is in a car seat with only a 3 point harness and is dated July 2004. My son is in a booster seat with a strap that he must sit on and the seat belt is weaved into it prior to being buckled into the car seatbelt lock. There is no shoulder harness to hold the seatbelt in place over his right shoulder so it moves up and down and provides no restraint what's so ever. He can sneak under the strap and reach across to his sister and pulls his arm in and out as he pleases no matter how many times we tell him it is not safe and why he should keep his arm under.

So my point! Yes, I believe there should be restrictions and guidelines for car seats, and I am grateful for the Canadian standards being so high, but why every country is so vastly different is beyond me. Why are perfectly good car seats tossed when I believe that they could be re-inspected and sent to a third world country to help protect those children less fortunate? And I do realize that in some countries, motorbikes are the normal mode of transportation and a car seat wouldn't work in that case. Many countries you normally just see moms with a baby in a sling strapped to the front and a tiny tot holding on tight as can be behind, sometimes there is even more than one on the back. I am sure we could make better use of car seats that could go through an inspection process and be redistributed. I would like to find some country that would be happy to have these donated car seats for their children to have some type of safety.

So my questions for you to ponder is are we so safety conscious that we are willing to fill our landfill with product that could fit into the 3 R's and be reused or am I out to lunch in thinking that they could never be good to anyone.

Low Back Pain And Sciatica - Functional Limitations

The problem with low back pain and sciatica, is that the lower back itself and surrounding area is so important with regards to any movement or activity you become involved in. It is not like when you have injured an arm or a leg, where you can rest the arm in a sling or use some crutches for your leg in order to allow healing to take place, with low back pain you are not afforded that luxury.

With regards to functional activities, the first thing which needs to be addressed is to divide them into two categories:

i) Those which cause or aggravate your low back pain or sciatica.

ii) Those which cause you no pain or do not increase your pain at all.

The chances are that there is a common factor linking the components of each of these two groups.

For example, let's say sitting down aggravates your low back pain yet keeping upright and a small amount of walking eases your pain. We are going to be thinking along the lines of flexion activities being the aggravating factor for your low back pain and extension type activities being the easing factor.

With this above example, first and foremost we need to modify or temporarily avoid the aggravating factor i.e. sitting down. Now I am not suggesting you do not sit down at all during the day, but rather you try and sit in a more upright chair, with say a rolled up towel for some support for your lower back. In addition to this, I would also recommend that you spend no longer than a maximum of 15-20 minutes sitting down without break i.e. getting up out of the chair.

Moving on to encouraging the easing factors, and once again using the above example, I would suggest you try to have a little walk a few times during the day. This can be anything from a few minutes to say 20 or 30 minutes, depending upon the severity of your pain. The important thing is your low back likes this activity and therefore it is important to provide it with it, as it will be providing you low back with the optimum conditions for it to heal itself.

NB. You can get too much of a good thing, therefore do not just walk for the sake of it. For example, if your low back pain eases after 5 minutes walking, but becomes aggravated after 10 minutes, feel free to walk for 5 minutes, but not too much longer (otherwise you will be simply interfering with the healing process again).

With regards to aggravating and easing factors, it is important to be as specific as possible. Therefore,using the example I have just given, walking for 5 minutes is an easing factor, yet walking for 10 minutes is an aggravating factor.

The main principle is little and often throughout the day. Tying in the above two, the logical thing to do with regards to sitting, is every 15 minutes or so stand yourself up and have a walk up and down the living room. By taking on board the above principle, it will result in less stress being placed across the structures responsible for your low back pain and therefore give your body a chance to heal itself. As healing takes place, the structures responsible for your pain will become stronger, and if they are stronger you will find you can begin to do more and more.

A positive cycle will now result, whereby encouraging the easing factors will promote more healing, which itself will result in your low back becoming stronger and therefore less easily aggravated. If it is less aggravated, it means more healing will take place and therefore you can become more active and your back will become stronger.... and so on.

Sound too simple?

Well there is no doubt I have simplified things a little just to highlight a point. However, it is not too difficult either as the underlying principle is the same. The all important aspect is firstly analysing your day-to-day activities and then dividing them into aggravating and easing ones. When you have established this, begin by temporarily avoiding/modifying the aggravating activities and encouraging the easing ones. As you do this, your low back pain or sciatica will begin to ease and you will be able to become more active.

At this stage, it will be important to consider addressing any muscle imbalances which are present (as it will almost be certain there are some). A simple but effective exercise programme to strengthen any weak muscles and stretch any tight ones will soon have you performing those functional activities you are finding difficult at the moment pain-free...

It can be caused by arm strain due to lifting heavy loads, it can be the result of joint strain or rotator cuff injury, and it can be something as simple as sleeping in a position that causes you to strain or pinch your arm at night.

In addition to the arm pain, you might experience swelling or stiffness. What can you do to alleviate your arm pain and other symptoms without having to resort to powerful synthetic drugs?

First, just rest. Refrain for at least a couple of days from any activity that might stress and further strain your arm. If you are not sure which activities are straining your arm like that, err on the side of caution--don't worry about being "lazy". Any activity that you suspect, cut out for at least a couple of days.

You can also try to apply ice to the sore area of your arm. Apply an ice pack or simply wrap ice cubes in a towel and apply that. You should especially do this if you experience swelling in your arm. You should apply the ice for 15 or 20 minutes at a time, at least three times per year. For the first two days that you are doing this, don't do or ingest anything that could cause or aggravate inflammation--hot showers, drinking alcoholic beverages, drinking coffee, etc. Some of the things you should avoid for these two days might be a regular part of your daily life, but you can go for 48 hours without them. You should also not smoke for the duration of the time that your arm is in pain.

After this period, assuming the swelling is down, you can start applying heat to your arm pain, such as heat pack. You can also begin doing easy exercises to get your arm's tendons and muscles stronger again so that you prevent future injury. Don't forget to remove all of the jewelry on that arm and that arm's hand if you experience swelling, so that you minimize the chance of strangling or diminishing blood flow.

If swelling continues, try wrapping your arm in a compression bandage. If you experience tingling or your fingertips get very cold, the bandage is too tight and you need to loosen it. Keep it on for up to three days and see if your swelling goes down, and if it doesn't go see a doctor.

Try to elevate your arm when you apply the ice every time you have a chance. Elevate it above your heart so that you will stimulate more blood flow to it and therefore minimize swelling. It's okay to get and wear a sling if you feel that this is helping you.

Massage your arm gently and frequently for five to 10 minutes at a time. However, if it causes more pain for you to do this, don't do it. This might mean you have a serious injury, more than just arm pain, and you should see your doctor.

Still, even when arthroscopic surgery is used, many shoulder surgery patients experience a burning pain after shoulder surgery. This can last for days or weeks and can trouble the patient, who is just trying to get on with his or her life.

Here are some tips for how to handle burning pain after shoulder surgery. Note that the best course of action for you to reduce and eliminate your own shoulder pain will vary by the shoulder problem being corrected and the choice of procedure. Medications:

Immediately after surgery, you will likely be given strong pain medications at the hospital, such as morphine or Demerol.

During surgery, your doctor may have chosen to insert a small plastic catheter into the actual site of the surgery inside the body. Immobility and Physical Therapy:

You will likely need to keep your shoulder largely immobile for a period of weeks, using a sling. It is important to not move your shoulder during the indicated period, even if the pain goes away.

Your doctor may also prescribe physical therapy that you can perform at home. Examples include: the pendulum (allowing your arm to swing in small circles), isometric exercises (tightening the muscles), passive range of motion exercises (whereby someone else moves your arm), and resistance exercises (whereby the shoulder is moved against resistance). Following these faithfully should ensure smooth healing and reduce pain.

Q: What is an autopsy, and why would it help my case?

A: An autopsy is an in-depth examination of a dead person, by a doctor. The doctor who performs the examination is usually a pathologist who looks to find the precise cause of death. They do this by looking at all of the internal organs, including the brain, heart, lungs, liver, kidneys, and spleen. Each area of the body is examined for evidence that contributed or caused that person's death.

In a case involving claims of wrongful death (where a person or family has claimed that their loved one died because of someone else's carelessness) having an autopsy is crucial to proving your case. While an autopsy is vital to support such a case, it can also shed light on the possibility that your loved one did not die as a result of wrongdoing.

It's a double edged sword. The autopsy could help your claim by showing that your loved one died from wrongdoing, or it could show that the treatment or actions that happened before death did not play a role in causing the death.

There are some religions that prohibit autopsies, and in those cases, it becomes extremely difficult to prove, with a reasonable degree of probability, that wrongdoing (such as malpractice) caused their death. In those cases, we must rely on other evidence to support our claim.

I am often called upon by grieving families to ask whether an autopsy should be performed on their loved one. As in life, there are no set answers to this crucial question. The most common case where an autopsy is performed is in a traumatic accident. In murder or homicide cases autopsies are always performed as the police want to know exactly what caused the person's death. They can usually use this information to track the perpetrator.

In New York, if a person dies suspiciously, or within 24 hours of having had surgery, an autopsy will usually be performed to determine the precise cause of death.

For example, I had a case where a man on dialysis came home one day, and was found later by his family in his bathroom having bled to death. The walls were covered with blood and there were open bandages all over the floor. An autopsy was able to confirm that the man's shunt (the place where the dialysis needle was put into his arm each session) had gotten infected and progressively larger with each session. Nobody recognized that he was starting to bleed when he left the dialysis center. Unfortunately, when he arrived home, the shunt ruptured and since it was connected to an artery, blood shot out all over the bathroom, creating what looked like a murder scene. It was only through the autopsy that we were able to prove our case successfully.

Autopsies are usually performed by the County Medical Examiner. In the five boroughs of New York City, Brooklyn, Bronx, Queens, Manhattan and Staten Island, autopsies are performed by the New York City Medical Examiner's Office. In Nassau, it's the Nassau County Medical Examiner, and in Suffolk, it's the Suffolk County Medical Examiner.
